Your Assignment Will Include:

• Provide individualized outpatient nutrition counseling for residents

• Develop and implement nutrition care plans based on medical needs and preferences

• Respond to nutrition consults from medical and nursing staff as needed

• Collaborate with interdisciplinary teams to support resident wellness

• Document assessments, interventions, and outcomes in electronic medical record system

• Support initiatives that promote healthy aging and quality of life

• Maintaining confidentiality and patient information in accordance with HIPAA

Requirements:

• Credentialed as a Registered Dietitian Nutritionist (RDN) through the CDR
• Must pass a pre-employment screening process, including background and health checks
