Registered Dietitian – Part-Time | Lebanon, NH

Nutrition That Works, LLC is seeking a registered dietitian to support a long-term care facility in Lebanon, NH. This opportunity starts in July for 12-18 hours/week onsite.

• Location: Lebanon, NH

• Schedule: 12-18 hours/week onsite 

• Duration: Open-ended

• Facility Type: Long-term care facility

• Drive Time: Offered for qualifying candidates

• Experience: LTC and MDS experience preferred

Your Assignment Will Include:

• Completing nutritional assessments and maintaining documentation
• Documenting with the Nutrition Care Process in the electronic medical record system 
• Collaborating with the interdisciplinary care plan team and food service team 
• Calculations and recommendations for enteral feedings 
• Making recommendations for wounds and significant weight changes
• Completing MDS - Section K
• Maintaining confidentiality and patient information in accordance with HIPAA

Requirements:

• Credentialed as a Registered Dietitian Nutritionist (RDN) through the CDR
• Licensed in New Hampshire (must be active prior to start date)
• Must pass a pre-employment screening process, including background and health checks
